common disorder which may have both mental and physical effects.l'2 It is an excellent example ofthe close interaction between respiratory and cardiac function. It is claimed that among SAS patients there is an increased risk of sudden death during sleep.3 The literature which states this is very limited, but it seems that these patients run an increase risk of developing systemic hypertension, cardiac diseases and strokes.4-6 The main pathophysiological mechanisms that are implicated in the haemodynamic changes during sleep in patients with SAS are firstly the cessation of breathing and the consequent hypoxia, and secondly the development of extreme negative intrathoracic pressures (obstructive apnoea). These two mechanisms which differentiate patients with SAS from other groups of patients who experience severe hypoxaemia during sleep but without any cessation of breathing,7 lead to a reduction in cardiac output due to

Cheyne-Stokes respiration and supine dependency.

The influence of position during sleep on central apnoeas during Cheyne-Stokes respiration has not previously been studied systematically. The current authors aimed to study the effect of body position and sleep stages on central sleep apnoeas during Cheyne-Stokes respiration. A total of 20 consecutive patients with cardiovascular diseases and central sleep apnoea during Cheyne-Stokes respirati...

Non-invasive beat to beat arterial blood pressure during non-REM sleep in obstructive sleep apnoea and snoring.

BACKGROUND Obstructive sleep apnoea, and possibly snoring, are associated with a poorly understood increase in cardiovascular mortality which may be explained by their effects on systemic blood pressure during sleep. This study compares changes in mean blood pressure during obstructive sleep apnoea and snoring without apnoeas with those in matched control subjects during non-REM sleep. METHOD...

Changes in heart rate during obstructive sleep apnoea

A paper was recently published in the Jownal (1] concerning heart rate (HR) in obstructive sleep apnoea In this paper a new hypothesis was put forward to try to explain HR changes in obstructive apnoeas. According to that hypothesis, upper aiiway receptor stimulation in apnoeas could activate postinspiratory neurons; postinspiratory time could then increase and this, in turn, could contribute t...

Respiration during sleep in neuromuscular and thoracic cage disorders.

Many of the neuromuscular and thoracic cage disorders are associated with disorders of breathing during sleep. The abnormal mechanics of the chest wall impairs respiratory muscle function and this is compounded if there is underlying muscle weakness. Respiratory abnormalities appear during REM sleep before NREM or wakefulness. Central sleep apnoeas are characteristic, but obstructive apnoeas ar...

Arousability in sleep apnoea/hypopnoea syndrome patients.

Sleep disruption and daytime sleepiness in obstructive sleep apnoea/hypopnoea syndrome (OSAHS) patients result from recurrent apnoeas/hypopnoeas and arousals from sleep. Around 30% of apnoeas/hypopnoeas are not terminated by visible cortical arousals. The current authors tested the hypotheses that arousal induction is linked to sleep stage, oxygen desaturation, event type, event duration and ti...

Spectral oscillations of RR intervals in sleep apnoea/hypopnoea syndrome patients.

A recent study has shown that daytime heart rate variability is reduced in obstructive sleep apnoea/hypopnoea syndrome (OSAHS) patients. In the present study, the hypothesis was that sympathovagal balance around apnoeas/hypopnoeas and nocturnal autonomic activity are altered in OSAHS patients. Frequency- and time-domain analyses of RR intervals were performed to monitor sympathovagal activity n...
